Under "Shared bookings pages" on the bottom right hand side of the page select "Create booking page" 
On the initial option menu select "Create booking page from scratch"
On the next screen, if you are doing this alone, use your name for the name of the page. Bookings is creating an email linked to the name there for appointment scheduling. I do not think you can change the email, at least not easily, after this stage so choose wisely. If you want to put a business type, I suggest Education.
Choose who can see your page and hit "create" to create your shared page.
